Your PMB condition will usually require you to use medicine. To register your medicine for PMB, simply follow these three easy steps:
Download and complete the medicine application form
Take the form to your attending doctor to complete and sign. You will see that the application form contains sections for your doctor to complete regarding your chronic illness. The application form may also require that test results be attached (this does not apply to all conditions, so please check first to see if it is required for your specific condition). These test results will confirm whether your condition meets the entry criteria to qualify for PMB.
Fax the completed application form to (012) 334 2466 or post it to PO Box 26042, Arcadia, 0007. Alternatively scan the completed form and e-mail it to medihelp@medihelp.co.za.
